Homesafe Plus

Patient information

Patients should be medically fit to be discharged from hospital, but would benefit from some additional support via the Homesafe Plus services in the first weeks after discharge. They must live in Elmbridge, Runnymede, Spelthorne, Woking or Surrey Heath.

Ideally the patient should have an emergency contact, preferably one who resides close by to assist the patient if necessary. However if no emergency contacts are available, the patient must have an existing key safe or take home a temporary key safe.

If the patient already has a community alarm or is receiving meals, it's worth checking with their borough council first whether any additional assistance can be offered.

Once the patient is home, any equipment will need to be set up. If the temporary key safe is being used this will need to be set up by inserting the code allocated to the key safe, putting in a key to their home and locking it to a post or railing outside. Secondly, the community alarm will need to be plugged in to the landline telephone socket, the alarm switched on and the button pressed to connect with the Homesafe Monitoring Centre. Instructions are located within the pack illustrating these procedures. Please ensure there is someone able to set these up otherwise the alarm will not work properly.

The initial support provided through Homesafe Plus is free of charge to the patient. Once they have received contact from their local borough council, they will be informed if there is any charge for the services they would like to continue with.
