After a referral has been made

When you complete a referral form you will be directed to give the patient an Emergency Homesafe Pack if one is needed. Please do not give the patient an Emergency Homesafe Pack if you are not directed to. The borough council will arrange installation of a permanent alarm.

If the patient is being given an Emergency Homesafe pack there is a pre-programmed key safe in the pack for them to use.

For all other scenarios where a key safe is required one of the pre-programmed key safe only packs should be used. There are three scenarios in which a patient should be given a key safe only pack:

  • Emergency pathway - needs a community alarm but doesn't have a landline or an existing key safe
  • Emergency pathway - needs a key safe but does not need a community alarm (or already has one)
  • Referred to Meals at Home but patient is unable to answer their door and doesn't have an existing key safe

If you are unable to locate a pack at the hospital it may be that they are all in use. Please check all appropriate locations and please do not submit a referral without first locating a pack.

The patient will be contacted within two working days of discharge, usually in the form of a telephone call. This is an initial wellbeing check to ensure that the patient has no immediate concerns or to arrange follow up appointments. Within five working days it is aimed patient will receive a home visit from the council
