- Who are we and what services do we provide?

Homesafe Plus is a service provided by your local council, in association with The North West Surrey (NWS) Alliance, to provide assistance following your discharge from hospital and to support you to live safely and independently at home. The service is delivered in partnership by the following councils:
- Elmbridge Borough Council
- Runnymede Borough Council
- Spelthorne Borough Council
- Surrey Heath Borough Council
- Woking Borough Council
Each council is responsible for the data of their residents however we work together to ensure that this service is as efficient as possible. For example, the online referral form is hosted by Runnymede Borough Council via their website.
- What personal information do we collect?

We, your local council, collect and process the following information to provide you with the services you have been referred to through Homesafe Plus.
- Identity data (name, address, contact details, next of kin, emergency contact details)
- Biographical information (date of birth, gender)
- Personal circumstances (marital status, carer status, who you live with. We also ask about your home status if specific services are referred to e.g. Housing, Home Improvement Agency/Handyperson)
- Special category data (ethnicity, medical and health information including NHS number and GP details
- Personal information that may indicate a risk to self or others
- Care agency details
We, your local council, may collect and process some or all of the following information for monitoring and research purposes. This is to ensure this service is continuing to benefit the local community and to provide evidence to support the funding of the service.
- Name or NHS number
- DOB
- Gender
- Postcode
- Information about personal circumstances
- Special category data
- What council services you have interacted with and the outcome
We collect this information from your health or social care professional via the online referral form hosted by Runnymede Borough Council.
- What lawful basis is engaged?

Our lawful basis for collecting or using personal data to provide you with the services you have been referred to through Homesafe Plus:
- Public task - we need to collect or use your information to carry out a task laid down in law, which the law intends to be performed by an organisation such as ours. All of your data protection rights may apply, except the right to erasure and the right to portability.
Specifically, NWS Alliance have a duty to provide support and assistance to patients when they leave hospital and to ensure that health and wellbeing needs are met in the community. They have provided funding to local councils to support this duty, and the partnership councils have an agreement to provide the Homesafe Plus service to meet this objective.
Our conditions for processing special category data relating to this purpose:
- Explicit consent - the referral process requires explicit consent to transfer the data from the health and social care professional to their local council for the purpose of providing the services referred to.
- The provision of care services pursuant to contract with a health professional - we require information about your needs and health to provide the services that you have been referred to via Homesafe Plus as agreed with NWS Alliance.

- Who has access to your personal data?

Our processors use your data to provide us with services necessary to meet our objectives. We ensure that there are appropriate agreements in place to protect your data, and it is only processed under our instruction.
- Runnymede borough council who host the online referral form using Jadu and collate information for monitoring purposes.
- Safer Runnymede who store data in the Jontek database to provide monitoring services.

- Who do we share data with?

Where necessary for the purposes stated, your information will be shared with the following organisations:
- North West Surrey (NWS) Alliance - for research and evaluation, to support funding and improvements to service provision.
NWS Alliance is a health and care partnership organisation and includes Surrey County Council, NWS Borough Councils, Surrey Heartlands, Ashford and St Peters Hospital and local GPs. See the North West Surrey Alliance website for more information.
- Emergency services - we will provide your personal information to support your emergency care.
- How do we protect personal data?

We take the security of your data seriously. We have internal policies and controls in place to reduce the risk that your data is lost, accidentally destroyed, misused or disclosed, and inappropriately accessed. Data will only be processed by members of staff authorised by your local authority as the Data Controller for this purpose. Access to systems is limited to authorised members staff whose job role requires access to the personal data.
- How long do we keep your personal data?

Runnymede Borough Council, who host the online referral form, will delete the copy of the web form that sits in Jadu, the Cloud based IT system, after a retention period of 6 months and incomplete/unsubmitted forms will be deleted after 1 day.
Duplicate referral forms used by Safer Runnymede and for administration purposes will be deleted after a retention period of 1 year.
The data entered in the Jontek system used by Safer Runnymede, to allow for monitoring functions to be provided, will be retained for 6 years (or 2 years if deceased) in accordance with the data retention schedule, once transferred to the relevant borough council service provider.
Data held for monitoring purposes will be retained for 3 years after which time the personal identifiable data will be deleted, and the remaining data stored to monitor and evidence longer term trends and results.
Your local council will hold your personal data for 6 years after your engagement with the service ceases to provide a record of your interactions with the service and document the consent we received to process your information.
