The form will ask for some information about you. Most of the information is optional. However, if you provide us with this information it will help us select a representative cross-section of Panel Members. The only information which is mandatory is:
- Your name (if a resident) or your business name (if a business)
- Address, including postcode
- Daytime telephone number
- Email address
