Licensing forms information
The different licensing application forms are listed below. Due to payment facility upgrades the online application process via Gov.uk is currently out of action, please use the downloadable forms below till further notice and submit either by post or email, to pay for your application call 01932 838383 during normal office hours.
Please note that if you are submitting a new premises application or a full/minor variation there are strict requirements around the plan you must submit and advertising of your application. 
LA2003 - Plans and advert information
The advertising requirements include the placing of a notice at the premises. These must be on a mid blue coloured paper for new licence applications and full variation. They can be on white paper for minor variations. These notices along with a template newspaper advert are shown below.
